Why a two-phase approach?

We are intentionally opening our practice in two phases to ensure high-quality care while we continue working full time outside the practice.

Phase One - Foundational Launch (current)

What this means:

  • Limited appointment availability

  • Limited Primary Care membership spaces

  • Appointments offered during scheduled clinic blocks

  • Portal messaging for non-urgent concerns during business hours

  • Focus on quality, continuity, and sustainability

This approach allows us to provide thoughtful care without overextending access during the early stages of the practice.

What to expect

During Phase 1, we ask patients to review the following expectations so we can provide safe, thoughtful care.

Appointments

  • Care is provided by appointment only

  • Appointments are offered during scheduled clinic blocks

  • Same-day visits are not guaranteed  

Communication

  • Secure portal messaging is available for non-urgent questions

  • Messages are reviewed during business hours

  • Response times may vary based on clinical volume

Portal messaging is not intended for emergencies.

Primary Care Membership

  • A limited number of Primary Care memberships are available

  • This allows for longer visits and individualized care

  • Membership availability may change as the practice grows

Visits, Labs, and Medications

  • Visits are required before starting or adjusting medications

  • Labs and medications are not included in visit or monthly fees

  • All options are discussed before ordering

Phase Two - Expanded Access (future)

As availability increases, Revera will:

  • Expand appointment availability

  • Increase Primary Care membership capacity

  • Improve access for urgent concerns

  • Add additional services and care options

We will clearly communicate when we transition to Phase 2.
